Forms for Central Kitsap Sailing Team

You will need a carpooling form for every traveling event, so you might want to print a bunch of them and have them ready. Sailors getting a ride need to fill in the bottom portion.  Parents driving sailors need to fill in the top two portions.  Sailors may not drive themselves to weekend regattas due to Parks Department regulations.  No form is required for sailors who ride with their own parent.
